A Path to Recovery and Independence
Transition House in Geelong is a safe, affordable, and supportive shared accommodation for men rebuilding their lives after addiction. Designed for men who have completed the six-month rehabilitation program at Foundation 61, Transition House offers short-term residency to help individuals reintegrate into the community and progress toward full independence.
Each resident benefits from the guidance of a live-in support person who provides essential practical assistance, fostering skills for independent living and social reintegration. In addition, every tenant is paired with a dedicated COACH mentor—a trained volunteer who offers friendship with purpose. Together, they work toward personal goals in areas like education, employment, mental health, addiction recovery, financial management, family stability, and overall well-being.
Through this unique program, Transition House helps men transition from recovery to independence, providing a foundation for long-term personal growth and stability.
